• How do I become a new client?

Download new client forms below; print and complete or if you choose you may fill them out at your first appointment. If you choose to fill out at your first appointment, please arrive 15 minutes early to do so. Click below for printable office policies and procedures.

• Do you accept insurance?

The only insurance company I participate with is Medicare. My billing office will submit claims and be reimbursed by Medicare and secondary insurances. Other than participating with Medicare, I am a fee-for-service clinic. This means that payment is due at the time services are rendered. I accept cash, credit cards and personal checks. As a courtesy, my billing office can submit claims on your behalf; please submit all insurance cards to the office if you choose to do this. I will also be happy to provide you with a detailed invoice for you to submit to your insurance company for reimbursement. It is your responsibility to know your physical therapy benefits. Check if prior authorization is required and follow up with my office if it was obtained and number of visits that were approved.

• Do I need a prescription/referral from a doctor?

Maryland is a direct access state, therefore you may attend physical therapy without a referral. If you have Medicare as your primary insurance, you must provide the name and telephone number of your physician so that the provider can be given your plan of care to sign and return.

• How long is each session?

Both the initial and follow up sessions are 1 hour long. For your first session, I will discuss your current condition or injury, including specific hands-on-tests to find out what is contributing to your issues.

• How many visits will it be?

After your evaluation, I will give you an estimate of how long I anticipate it will take to get you better and to meet your goals.

• What should I wear?

Basic athletic or workout attire or comfortable loose fitting clothing is appropriate and helpful.

• Who chooses where I go for Physical Therapy?

You are the most important member of your healthcare team and you have the right to choose who you want to provide your care to meet your goals.

• What do I do if I need to cancel an appointment?

In the event that you are unable to attend your scheduled appointment, please kindly call or leave a message with at least 24 hours notice prior to your appointment time. You may be asked to reschedule the missed appointment within the week if possible. Any appointment that is canceled with less than 24 hours notice or considered a No-Show will be charged $70.00 for that appointment time.
